Understanding POI

What is premature ovarian insufficiency?

Premature Ovarian Insufficiency, often shortened to POI, is a condition in which the ovaries function less regularly before the age of 40. It may be associated with irregular or absent menstrual periods, lower estrogen levels and reduced fertility.

POI is sometimes referred to as primary ovarian insufficiency. It is not exactly the same as menopause, because ovarian function may occasionally continue or resume intermittently in some people.

An early and sensitive assessment is important because POI can affect reproductive plans as well as bone health, cardiovascular health, emotional wellbeing and quality of life.

POI care looks at the complete picture

2 Menstrual history

4 Overall health

Your care plan should reflect both your current health needs and your personal plans for the future.
Recognising POI

Possible symptoms of premature ovarian insufficiency

Symptoms vary from person to person. Some people experience changes gradually, while others may notice menstrual changes more suddenly.

📅
Irregular or missed periods
💙
Difficulty conceiving
🌡
Hot flushes or night sweats
💧
Vaginal dryness or discomfort
🌙
Sleep or mood changes
⏳
Reduced sexual wellbeing
Some symptoms can also occur with other medical conditions. A medical assessment is needed to identify the cause.

Possible causes and associated factors

POI can have several possible causes. In many cases, a clear cause is not identified.

Possible contributing factors
  • Genetic or chromosomal factors

  • Autoimmune conditions

  • Previous chemotherapy or radiation treatment

  • Ovarian surgery

  • Certain infections or medical conditions

  • Family history of early ovarian insufficiency

Why assessment matters

Your specialist may recommend investigations based on your age, symptoms, family history and medical background. Identifying relevant factors can guide appropriate fertility counselling, health monitoring and long-term care.

POI is not caused by anything you did or did not do. Supportive, respectful care is an important part of management.

How is POI assessed?

Diagnosis involves a clinical assessment and may include more than one test, particularly when menstrual changes are recent or causes need further evaluation.

Whole-Person Care

Care for health beyond fertility

POI management is not limited to fertility planning. Reduced estrogen can have implications for bone health, cardiovascular health, sexual health and emotional wellbeing. Your care plan should address your individual symptoms, long-term health and reproductive goals.

Family-Building Pathways

Fertility options with premature ovarian insufficiency

POI can significantly reduce fertility, but fertility counselling should be individualised. In some cases, ovarian activity may be intermittent. A fertility specialist can help you understand available pathways based on your diagnosis, health, treatment history and family-building goals.

Individual fertility counselling
Understand what POI may mean for your reproductive plans, timing and available options.
→
Fertility preservation discussion
Where ovarian function is at risk or before treatments that may affect fertility, preservation options may be discussed when clinically appropriate.
→
IVF using your own eggs
In selected circumstances, IVF planning may be discussed if ovarian activity is present. Suitability and expected response vary.
→
Donor-egg IVF
Donor-egg IVF may be one possible family-building option for some individuals with POI. This requires detailed counselling and personalised medical guidance.
→
No treatment can guarantee restoration of ovarian activity or fertility. Your care team can help you understand options with clarity and compassion.
