What Is Premature Menopause?

Premature menopause, also known as premature ovarian insufficiency (POI), happens when the ovaries stop working normally before the age of 40. This means periods can become irregular or stop, and the levels of hormones like oestrogen fall earlier than expected.

Some women with POI still have occasional ovarian activity, meaning hormone levels and periods can fluctuate rather than stop all at once. This is different from natural menopause, which typically occurs around age 45 to 55.

Key point: Premature menopause affects roughly 1 in 100 women under 40. Early diagnosis and personalised care can help manage symptoms, protect long-term health, and clarify fertility options.

Why Can Premature Menopause Happen?

In many cases the exact cause isn't identified, but several factors are known to contribute.

Some cases of premature menopause are linked to genetic and chromosomal factors, including conditions such as Turner syndrome or Fragile X premutation, which can affect how the ovaries develop or function over time.

Chromosomal conditions affecting ovarian development
Family history of premature menopause
Certain inherited gene variations

Certain medical treatments, particularly those used to treat cancer, can affect ovarian tissue and lead to premature menopause, either temporarily or permanently depending on the type and dose.

Chemotherapy affecting ovarian reserve
Radiation therapy near the pelvis
Surgery involving the ovaries

In some cases, the immune system mistakenly targets ovarian tissue, which can disrupt normal hormone production and egg release over time.

Autoimmune thyroid conditions
Autoimmune adrenal conditions
Immune-related ovarian inflammation

Certain lifestyle and health factors are associated with earlier onset of menopause, though they don't affect everyone in the same way.

Smoking
Very low body weight
Certain viral infections or environmental exposures

How Is Premature Menopause Diagnosed?

Diagnosis is a step-by-step process designed to understand your full picture before recommending a path forward.

1
Medical and menstrual history
A detailed conversation about your cycle patterns, symptoms and family history.
2
Physical examination
A general health check to help rule out other possible causes.
3
Hormone testing
Blood tests to measure FSH, estradiol and related hormone levels.
4
Pregnancy testing (where appropriate)
Used to rule out pregnancy as a cause of missed periods.
5
Pelvic ultrasound
An imaging scan to assess the ovaries and uterus.
6
Additional tests as needed
Genetic or autoimmune screening may be recommended in some cases.
7
Fertility and health discussion
A thorough conversation about what your results mean and what comes next.

Looking After Your Long-Term Health

Lower hormone levels earlier in life can affect more than fertility. Ongoing care helps protect your health for years to come.

Bone health
Lower oestrogen can increase the risk of reduced bone density over time, making monitoring and support important.
Cardiovascular health
Earlier hormone changes can affect heart health, so regular check-ups are part of ongoing care.
Sexual and vaginal health
Support is available for comfort and intimacy, including options tailored to your needs.
Emotional wellbeing
An earlier-than-expected diagnosis can bring up complex feelings, and support is always available.
